Csv Adding Filename As Passcodes

Good afternoon!
I've been trying to add these passwords to a gallery using the Older Passcode Method with a CSV. The first CSV screen shot below worked perfectly, no problems. When I ran the second CSV, it added the filenames as the passwords?
1st CSV worked perfectly
2nd CSV added Filenames as Passcodes
Screen shot of how the passcodes are appearing in my gallery.
Any assistance much appreciated!!!!

Is there a column before the Filename column in the one not working?
