Verify Qr Photos Glitch

I'm curious if anyone else is experiencing this. 

I uploaded photos using the QR code passcode system. (Saved me so much time over manually naming each student's images. Thank you, Tim!!!) 

Some of the codes didn't read properly as the image was at a poor angle or focus wasn't locked in, so I needed to select the code photo and student images, and tell the system what the passcode is. 
This worked to rename the image, but the QR code file is still hanging out in the previous row, appearing to be coded to the previous student as one of their photos. 
When I scan the code of the previous student, only their image shows so the QR code photo is as it should be, but from the admin side it doesn't look this way.

If you refresh the page, does it change?

That is what it sits at after refreshing, or logging out and going back in.

Immediately after recoding, this is what it looks like.

I should note that I had to recode a handful of students, and only two of them are doing this. The rest sorted into the proper rows after recoding.
